Booking Non-Emergency Ambulance & NHS Patient Transport
This is available for patients who have a clinical condition that prevents them safely making their own way to hospital. Please contact the Patient Choice/Referral Management Centre in East Lancashire.

Non NHS work: Medicals, Private Insurance Certificates, Claim Forms, Letters requested for Outside Agencies.
These take 5 working days to be completed but longer if the doctor is on leave. There is a charge for this service and if there are demands on the practice to complete earlier this can be arranged at an additional premium.
Test Results
Most test results are available between 4-7 working days. Please telephone for these results between 1.30 and 3pm. Cervical smear results take up to 8 weeks, and X-ray results take about 12 days from time of the hospital appointment.
Access to Referral/Letter from other Doctors
If you are aware of a referral/letter being sent regarding yourself from November 1991, you can request access to read it free of charge.
If you would like a copy of the letter, a nominal charge will be made to cover administrative costs.
Suggestion Box
We encourage patient participation and suggestions/comments toward service improvements. If you use the suggestion box, which is located at the reception desk, please leave your name and contact number in order that we may respond directly.
Patient Participation Group
If you are interested in being involved in the dynamics of the practice and wish to participate in improving/developing services by representing the patient body we welcome you to join this group. Please write in or mention it to a member of staff.
Complaints
We have an in-house complaints procedure. Please ask for a leaflet at the Reception desk.
Removal from the Practice
You can leave the practice to join another practice at any time and no explanation is required to be given by you. The practice reserves the right to give a warning about potential removal from the list, either verbally or in writing if there is unacceptable behaviour. If the behaviour continues, you will be removed from the list. Physical or verbal aggressive threats or violence towards any member of the Practice or other patients will result in instant removal.
How to Register
Please ask the reception staff for details. You will need to complete a health questionnaire and see the Health Care Assistant for a brief health update check, following which you will need to make a double appointment to see the doctor. The reception staff will assist you with completion of the questionnaire if required. Please notify us if you have any special needs e.g visually or hearing impaired or are unable to read/write.
